If the minor has been adopted, we will also need the adoption paperwork.

We understand that it can be difficult for a child to have an ID, but it is required by Texas state law. The law is in place to protect the child and ensure that the person bringing them to be pierced is, in fact, their parent or legal guardian.

  • Birth certificate for minor (must be the original; copies or digital copies will NOT be accepted; If the minor or guardian’s name has changed and no longer matches the way it is presented on the Birth Certificate, supplemental Marriage Certificate, Divorce papers, or Court Order of Name Change are required)

  • ID for minor (passport, school ID, drivers license)

  • ID for parent
